  • No tapering is required during discontinuation
  • Tablets should be swallowed whole and should not be cut, crushed, or chewed. Tablets should be taken with food
  • Patients with renal impairment: Doses of Horizant® must be adjusted in accordance with renal function

*Patients with a creatinine clearance <60 mL/min require a different dosing regimen.1

If the recommended dose of Horizant® is exceeded, reduce the dose to 600 mg/day for 1 week prior to discontinuation to minimize the potential for withdrawal seizure.1

600 mg QD days 1-3, then 600 mg BID thereafter

Horizant® offers twice-daily dosing and reaches therapeutic dose within 4 days1

  • The recommended starting dose of Horizant® for PHN is 600 mg in the morning for 3 days, then increase to 600 mg twice daily on Day 4
  • Horizant® is not interchangeable with other gabapentin products, because of differing PK profiles
  • Patients with renal impairment: Doses of Horizant® must be adjusted in accordance with renal function

PHN dosing in special populations1

  • Renal impairment: The dose of Horizant® should be adjusted in patients with renal impairment
  • Geriatric use: Elderly patients are more likely to have decreased renal function, so the frequency of dosing may need to be adjusted based on calculated creatinine clearance in these patients
  • Use with central nervous system depressants: Monitor patients who are co-administered Horizant® and CNS depressants, including opioids, for symptoms of respiratory depression and sedation, and consider initiating Horizant® at a low dose
